Try freshmeat.net search for 'tftp'.

I have no experience with most of the results, so I can't help, but I 
have seen / do see some servers that claim functionality similar to what 
you are requesting.

ttftp server, uses php, which would allow mysql (I presume)

tftp-hpa-execute which uses scripts? I presume you could use a C program 
or some such with mysql access.

DOCSIS Server, which is designed for Cable modems, but is a tftp server 
with mySQL backend, presumably could be hacked for your needs.


I am sure I have seen others, google may be able to help as well?
