Sorry, it should have work with patch only before.
I had issue unrelated to syslinux and in the battle, I probably forgot MENU
NOSAVE at top.
Don't know how as I remember to edit extlinux.conf for that change.

All is well in 3.75-pre3
Tested with MENU SAVE and MENU NOSAVE at top menu
Both work as expected.
