Pxelinux needs PXE.

You can get a floppy bootable Etherboot image from www.rom-o-matic.net,
if Etherboot supports the NIC.

ISC DHCP v3 server has conditional configuration which can fairly easily
allow configuration dependent on MAC address or anything else in the
DHCP request.
