... contains the latest version that I know of (eltorito.asm.ref is
Bart's code, eltorito.asm is my initial attempt at converting to NASM.)

Unfortunately it turns out this source code is somewhere between version
1.2 and 1.3.

Someone, but I can't remember whom, was working on trying to reconstruct
the changes from 1.4 back into the source code (which unfortunately
means we still don't have the comments and therefore know why the
changes were made, but Bart has lost the source to that version.)
