Find attached a reworked version of that patch

Changes:
 - renamed hexdata into isohdpfx
 - Makefile closer to Makefile in git HEAD
 - Makefile uses isohdpfx.{head,tail} to avoid further changes to
   isohdpfx.c being done in the Makefile
